Disease Registry & Care Management Solution
CareSentry is a disease registry and care management system.  Designed to work in concert with the NextGen EHR (EMR), CareSentry queries the clinical data contained in the NextGen EHR (EMR) based upon practice defined qualification criteria and then optimizes the data by utilizing it to provide quality care monitoring, real time alerts at the point of care, patient outreach, population management and reporting.   CareSentry is an integrated real-time quality care monitoring system designed to notify providers and health care personnel of scheduled orders and procedures.  CareSentry also monitors insurance and visit specific documentation requirements.

CHRONIC DISEASE REGISTRY
CareSentry helps identify and monitor patients that are at risk or have been diagnosed for specific conditions based on evaluation of their EHR data against a set of risk profile data points and monitors criteria entered into the “Manager” component of the software.   ·

-  Monitoring criteria is highly customizable.  Queries any NextGen EHR (EMR) template or template field (Custom or KBM).  This includes data in the templates that come from various interfaces i.e.  electronic lab results, ekg, etc.
-  Demographic, diagnosis and medication parameters from a defined “profile” (i.e. diabetes, hypertension) can be used to identify patients that match that profile for inclusion into the profile watch list.
-   There are no limits on the number of parameters a profile can have.

ALERTS AND DECISION SUPPORT

-   The CareSentry alerts appear as a small overlay toolbar at the top of each NextGen EHR (EMR) screen. This toolbar indicates pending items, warnings and completed items.
-    Scans continue to update the tool bar to alert clinical staff (real time) of any actionable items for the patient based upon the criteria established within the registry.   Throughout the visit, newly entered data may change the items within the list and indicate a new action to be addressed. 
-   The toolbar drop down provides both a graphical display of actionable items as well as the parameters being monitored
-  CareSentry provides an “auto add” ability to include patient in the registry when alerted in the chart

CareSentry can export data on patients requiring further action – visits, tests, labs, procedures so that the practice may reach out to them via letters and/or automated calling systems such as TeleVox and PhoneTree.

STANDARDS OF CARE

The primary focus of CareSentry is to help the organization maintain a consistent level of care across all providers. With the advent of pay-for-performance reporting (P4P) by Medicare and an increasing number of insurance carriers, the data collection monitoring provided by CareSentry will help ensure that the required information is collected consistently so that reimbursements are not negatively affected because of missing information.

CareSentry provides standardization of care across your organization for patients included in profile watch lists so that providers with varying levels of experience for different disease management plans conform to a consistent baseline for documentation and patient evaluation.
